Sliding Fee Scale

We at Horisons Unlimited Provide essential medical services to our patients regardless of their ability to pay. We use Sliding Fee Scale to calculate and offer discounted service to our patients depending on their annual income and the family size; this is updated annually using Federal Poverty Guidelines.
No one will be denied access to service due to inability to pay.
Once approved, the discount will be honored for six months, after which the patient must reapply.

Application process for Sliding Fee Scale Discount

A patient completes an application form and provides documentation of home address, household income, insurance coverage and other important document. Once the Business Office makes sure that the application is complete, it is approved after which discounted is granted. When applicable, a written denial of coverage by medicaid may also be required esp for those who appear to be eligible for Medicaid.
This Sliding Fee Scale is consistent with prevailing rate in community. For a patient at or below 200% of poverty, we offer a tier sliding fee schedule of 0%, 10%, 20%, 30%, 40%, 50%, 60%, 70%, 80%, 90% or 100%.
